Misplaced or Stolen Passport? Here Are the Required Documents for Replacement

Losing your passport can be stressful, especially when you need it for travel or identification. Fortunately, the Ministry of Foreign Affairs Ghana has outlined clear procedures and requirements to help you replace a lost or stolen passport quickly and efficiently.

Below is a complete guide to the documents you will need when applying for a replacement.

Mandatory Documents

To begin your application, the following documents are strictly required:

  • Printed and signed online application form

You must complete the passport application online, print it out, and sign it before submission.

  • Police Report (from the country where the passport was lost)

This serves as official evidence confirming the loss or theft of your passport.

  • National ID (Ghana Card)

A valid Ghana Card is required to verify your identity.

  • Birth Certificate

This confirms your citizenship and personal details.

  • Sworn Affidavit / Notary Declaration

A legal document stating the circumstances under which your passport was lost or stolen.

Additional Requirements (Where Applicable)

Some documents may be required depending on your situation:

  • Proof of Profession

This is needed in certain cases to support your application.

  • Copy of Parent’s Ghana Card or Passport (for applicants below 18 years)

Required to confirm parental or guardian identity.

  • Consent Form (if a parent/guardian is not accompanying a minor)

Ensures legal permission has been granted for the minor’s application.

Supporting Documents (Recommended)

Copy of the bio-data page of the lost passport (if available)

This helps speed up the verification process, though it is not compulsory.

 Payment Requirement

Processing fee payment receipt

Proof of payment must be submitted as part of your application.

  • Minors & Parental Consent

For applicants under 18 years, there are additional guidelines:

Parents or legal guardians must accompany minors to their appointment. In cases where this is not possible, a signed consent form and supporting identification documents must be provided.

Replacing a lost or stolen passport requires careful preparation of all necessary documents. Ensuring that you have everything ready before your appointment will help avoid delays.
