NSA Warns Against Unauthorized Payments for Monthly Evaluation Forms

 

The National Service Authority (NSA) has issued a strong warning to all National Service Personnel (NSP) and district offices concerning the illegal collection of money during the submission of Monthly Evaluation Forms.


In a circular dated February 2, 2026, and signed by the Director-General, Ruth Dela Seddo, the Authority disclosed that it has received serious reports that some NSPs are being asked to make monetary payments before their Monthly Evaluation Forms are accepted or processed at certain district offices.


No Payment Is Required

The NSA stated unequivocally that no payment whatsoever is required from any National Service Personnel for the submission, validation, or processing of Monthly Evaluation Forms.


According to the Authority, any demand for money in relation to Monthly Evaluation Forms is:

  • Unauthorized
  • Unacceptable
  • Contrary to NSA policies and regulations
  • Sanctions Already Applied


Management further revealed that some District Managers found culpable in similar cases have already been sanctioned (suspended) pending further investigations in line with the Authority’s disciplinary procedures.


This, the NSA emphasized, serves as a clear warning that such misconduct will not be tolerated.


Directives Issued

In view of the situation, the Authority has issued the following directives:


District Managers are cautioned to desist immediately from demanding or collecting money from NSPs in relation to Monthly Evaluation Forms.


National Service Personnel are strongly advised not to make any payments to any officer or individual for the submission or processing of their Monthly Evaluation Forms.


Any NSP who has been coerced into making payments is encouraged to report the matter immediately to the appropriate Regional Office or directly to the NSA Head Office for prompt investigation and action.
